TRANSITIONAL HOUSING SERVICES

This program is shared living space housing individuals for up to 2yrs, during which while participating in a Recovery Plan. Our Phoenix Rising Housing Project is a seamless continuity of housing services for up to two (2) years. Referral-based and/or have graduated from our Emergency Services.

Housed individuals must be:

• 18+
• Diagnosed with HIV
• Meet certain income guidelines
• Have a source of income per HUD guidelines (30%) pays toward rent.
