DAY PROGRAMS

Our groups include, but are not limited to Recovery Dynamics, Process Group, Pharmacology, Trauma-Informed Care, Sexuality Stage Model, Inner Child, Anger Management, Goal Setting, Health & Wellness, Healthy Sexual Behaviors, Budgeting, etc.

CONTINUUM OF CARE

HTL focuses on eliminating these disparities by collaborating with our Network of Community Health Partners to ensure that our program participants are connected to these partners as a seamless port of entry with ease of access that will lead to medication adherence and eventually viral suppression.

In addition to address various Social Determinants of Health i.e. Identification and employment challenges, justice system, transportation, food ect.

SUPPORT SERVICES

HTL provides non-medical case management to assist program participants with:

  • Birth certificates
  • IDs
  • Accessing public entitlements
  • Peer Buddy System
  • Peer Support Recovery Coaches

MEAL PROGRAM

HTL’s goal is viral load suppression in line with medication adherence. We provide daily healthy meals inclusive of Vegan and Vegetarians, nutrition education and access to cooking classes. Program participants can drop in for lunch.

BROTHAZ HELPING BROTHAZ

“Brothaz Helping Brothaz” is a new outreach program targeting MSM of Color (MSMOC) and Heterosexual Men of Color residing in Metro-Atlanta and the five neighboring counties. Our primary goal(s) is to identify MSMOC out of care and link them into care, identify HIV+ MSMOC with substance abuse and/or mental health challenges and link them into recovery programs, and educate HIV(-) MSMOC about PrEP and other prevention tools. In addition, build a network of the Brothaz in the south with goal to help other Brothaz. Our activities consist of utilizing outreach, recruitment, and linkage.
